  3. Provide Supporting Documents: This includes proof of identity (passport or birth certificate) and evidence of the end of any previous marriages (if applicable, e.g., divorce certificate or death certificate).

  4. Sign the Form in Front of an Authorised Witness: This could be your celebrant, a police officer, or another person listed on the form.

  5. Lodge the NOIM: Submit the completed form to your celebrant or minister, who will hold onto it until the big day.
